The Geriatric/Long Term Care Nurse Practitioner provides care delivery
and case management and is responsible for establishing and maintaining
effective relationships with patients, patients' families, key
stakeholders, facility staff and collaborating physicians.
In
this vital role, the Nurse Practitioner will work with a dynamic and
engaging team in a collaborative environment at long term care
facilities in the Fox Valley area in Wisconsin.
Education and/or experience:
Experience:
Must have 5 years plus experience as Nurse Practitioner. Geriatric or
long term care experience highly preferred. Experience with
electronic
medical record preferred.
Education:
Graduate from an NLN accredited Nurse Practitioner program.
Licensure/Certification:
Current RN license to practice in the State of Wisconsin and
certification as a Nurse Practitioner for Geriatric, Adult Health, or
Family Nurse Practitioner. Valid DEA.

are seeking a Family Nurse Practitioner or Physician Assistant to join
a well respected hospital’s newly renovated Observation Unit
Team
comprised of one Hospitalist and on Acute Care Nurse Practitioner. This
fast paced unit will test your abilities while offering a stellar
learning environment. The Provider will be responsible for rounding and
discharging of internal medicine patients in the Observation Unit as
well as assisting with admissions in the ED. One of the Providers would
work Mon-Fri 7:00am-4:00pm and the other Provider would work
11:00am-8:00pm with occasional weekend coverage. Must have at least one
year experience of direct Hospitalist experience preferred.
Compensation is negotiable depending upon experience with a starting
base salary of $95,000-$110,000+ incentive bonuses. The hospital
provides a rich benefits package including Medical, Dental, 401K with
